About Our Annual Wellness Visit

An Annual Wellness Visit (AWV) is a yearly health review for individuals that is part of a well-rounded preventative health plan. This service is available to you from the safety and comfort of your home through our expanded telehealth program.

The goal of the AWV is to improve your overall wellness with an emphasis on prevention. For Medicare patients, an AWV does not include a physical exam and is of no cost to you. 

You will leave with a personalized Optimal Aging Care Plan aligned with your goals and an interdisciplinary team to support you along the way.

During your Annual Wellness Visit, you and a member of your care team will:

	Assess your health risks		Review your family history		Work with an interdisciplinary team to develop a plan of care
	Screen for key areas of physical, brain, & social health		Conduct a comprehensive medication review
