About Our Advance Care Planning

Advance Care Planning (ACP) is a service where you discuss the type of health care you want if you become too sick to tell your health care provider. This service is available to you from the safety and comfort of your home through our expanded telehealth program.

For Medicare patients, ACP is a voluntary service that can be provided as part of your annual wellness visit, a benefit at no charge to you. We can also offer this service separately under Medicare Part B, where you may have a copay.

The goal of ACP is to ensure that you receive the medical care you prefer when you are not able to make your own decisions; care that aligns with your values, goals, and preferences.

The ACP visit will allow you and your primary care provider to know what your end-of-life wishes are and to complete the appropriate legal documents to support these wishes.

During your advance care planning visit, you and a member of your care team will:

Talk about possible types of
life-sustaining treatments

Explore options for sharing your
decisions with loved ones

Discuss types of treatment you would or would not want should you be
diagnosed with a life-limiting illness

 

Complete documents approved by your state that describe types of treatment 
you would or would not want
- and who you choose to speak for you -
should you be unable to speak for yourself
