About the Flourish Program

The Flourish Program delivers interdisciplinary patient-centered care coordination to adults with multiple chronic conditions living in rural Kentucky. This program uses health care navigation to link patients’ clinical & behavioral health care plans with comprehensive community care plans. This program is grounded in the Institute's Flourish Care Coordination Model. By addressing the physical health, behavioral health, and social determinants of health, this unique model of care improves health outcomes for patients and primary care practices.
