The Flourish NetworkThe University of Louisville Institute for Sustainable Health & Optimal Aging manages the Flourish Network, an initiative consisting of five coordinated programs that seeks to improve the health and well-being of older adults living in rural Kentucky communities. The programs of the Flourish Network are grounded in the Flourish Care Coordination Model, new way of understanding and approaching care coordination that links Clinical & Behavioral Health Care Plans and Community Care Plans. The Institute's Flourish Behavioral Health Internship Program funded by our HRSA Behavioral Health Workforce Education and Training grant. 2. iCCOA iCCOA (Interprofessional Curriculum for the Care of Older Adults) is our geriatric curriculum that trains students and professionals from medicine, nursing, social work, dentistry, pharmacy, and community health in the Flourish Model of Care. 3. Flourish Behavioral Health Graduate Internship The Flourish Behavioral Health Internship Program is a one-year, paid internship that trains students to provide care coordination and behavioral health interventions while working with older adults in rural Kentucky. Placements begin Fall 2018.
4. Kentucky Coalition for Healthy Communities Our community-based coalition that seeks to develop the social networks and resources needed to promote geriatric workforce development in rural communities and to increase the community supports that help older adults in rural counties lead healthier lives.
