The Flourish Network

The University of Louisville Institute for Sustainable Health & Optimal Aging manages the Flourish Network, an initiative consisting of five coordinated programs that seeks to improve the health and well-being of older adults living in rural Kentucky communities.

The programs of the Flourish Network are grounded in the Flourish Care Coordination Model, new way of understanding and approaching care coordination that links Clinical & Behavioral Health Care Plans and Community Care Plans.

The Flourish Network is supported through two HRSA grants. Learn more about the Institute's Flourish Network here.

With our HRSA Behavioral Health Workforce Education and Training grant, we are able to provide integrated behavioral health services to older adults in rural Kentucky through student education. Learn about our Flourish Behavioral Health Internship Program funded by this grant here!
